Much has been said about being “Trauma Informed”. I find it dumbfounds me that physicians deal with the effects of unresolved trauma in their patients (especially from childhood as noted in the Adverse Childhood Experiences, ACEs study in the late 90s) every single day in their practices, but most doctors know little about these Adverse Childhood Experiences and even less about their impact in creating chronic illness and even less than that about how to create subsequent effective treatment. Further, there is precious little education during a doctor’s training that would help us understand or prepare to meet the challenges of so many patients with ACEs. This lack of preparation and knowledge leads to immense frustration from our chronic pain patients, compounded by our own frustration as physicians when despite our best efforts, those patients fail to get better. You've likely heard the saying that “20% of your patients create 80% of your work and frustration”. Well, it is very likely these patients with ACEs are the 20%!

I have never seen a patient with Fibromyalgia or Irritable Bowel Syndrome, and to a lesser extent migraine headaches, who did not have significant ACEs. In over 20 years of family practice EVERY patient with IBS or FM I could track back to significant childhood wounding. Today I know that this is because when faced with trauma too much for their young minds to bear, their Autonomic Nervous Systems could not calibrate properly to the “normal” reciprocal activation (when Sympathetic is ON Parasympathetic is OFF)and instead developed an overprotective defensive and confused coactivation of both parasympathetic and sympathetic wings simultaneously. The result being the muscles and intestines getting both messages of activation and relaxation simultaneously, with the result of this discoordination of autonomic signals being the sensation of chronic pain. This is just one of the revelations I had in SE training.

SE training helped me understand my patients so much better. It helps me show them how to regulate, rebalance and recalibrate their nervous systems, but perhaps more importantly, it has shown me how to regulate and rebalance my own. I can tell you without a doubt that had I done SE training while I was a practicing family doctor that I would be given the emotional awareness and self awareness skills that I would have needed to prevent burn out. I was so out of touch with my own emotions, and along with denying my own emotional needs that OF COURSE I was destined burn out.

SE training will definitely help you understand your patients more, but the training is intuitive and creates a level of emotional intelligence few physicians ever reach. The reason so many doctors are feeling burned out is exactly this paucity of emotional intelligence and SE training specifically addresses regulation of your own nervous system.

To be blunt, that lack of emotional intelligence is why we burn out. SE training provides a level of self awareness and teaches you to be fluent in the language of the body and the emotions of that body. At an Interpersonal level, SE training helped me regain a sense of reward from helping my patients in a brand new way, while at the same time was invaluable at helping me at an Intrapersonal level in dealing with my own uncomfortable emotions, so I could begin seeing patients again in a way that was both fulfilling and sustainable.
